Pharmacology and American Drug Attitudes

on the limits of the scientific method

In reading about the history of pharmacology this morning, I discovered that it never occurred to the leaders in that field that the scientific method might not apply to certain subjects. Yet how can you apply the scientific method to the study of a psychedelic whose benefits depend on one's attitude toward those substances, as the indigenous might put it, or on the "set and setting," as American psychonauts might say? The scientific method tells us to limit the variables that we wish to study to a few important ones. But how does the researcher choose what's important when it comes to human behavior? Who died and made them God that they could make that call in the first place? Meanwhile, a litany of philosophical concerns arise when we start talking about using blinding in such cases. The researcher's attempt to avoid the placebo effect is highly problematic. Indeed, blinding is unethical in the case of the study of psychedelics, where the results of use are so obviously a joint production of both the user and the drug itself. To use blinding is to encourage "bad trips," since it forces study participants to encounter psychedelic effects without the requisite psychological and/or spiritual preparation.

Nor will researchers see the expected predictable correlations between dose and response when studying psychedelics, since a small dose may facilitate a profound response while a large dose may result in nothing -- all thanks to the fact that the user, for some reason that may be clear or mysterious, is not particularly susceptible to the drug's beneficial effects at the specific time of use.

The study of such drug use simply does not fit the scientific method, since even the self-same person when studied in the same way may yield results that vary dramatically over time.

And yet scientists, like Peter Simons of Mad in America, insist on deconstructing the psychedelic psilocybin by the scientific method, as who should say, "See? There's nothing here, never mind that those superstitious Mazatec thought it was a teacher plant of some kind, Lord alone knows why." 1 2 Simons holds the psilocybin mushroom to the same standards that he would apply to a laxative: he wants it to work in a specific quantifiable way every time. But this is not how psilocybin mushrooms have historically been known to work. And to hold them to those standards is just wrong. What Simons really seems to be asking is, "Can this shroom be reliably used to treat a specific condition?" and the answer to that question is always "no" insofar as those who ask such questions are ignoring the all-important circumstances of use that help give the drug its efficacy, including a limitless list of factors, such as the culture in which the user lives, their personal expectations, their goals in life, their goals in using, their feelings "on the day," their faith in the shroom's power (or lack thereof), etc. etc.... as well as a little luck, good or bad, or the blessings of fate, or the favorable intervention of the mushrooms themselves. (The famous curandera Maria Sabina lovingly referred to her psilocybin mushrooms as "colitas," little ones, or "santos," holy ones.)


To employ the scientific method in the study of mind and mood medicine is to assume that behavior is a simple affair and not, as the holist would say, the result of a vast array of complexly interacting inputs, none of which can be considered advisedly in the absence of its fellows. This is surely what Alfred North Whitehead had in mind when he complained that, "Scientific reasoning is completely dominated by the presupposition that mental functionings are not properly part of nature.” 3 This is the feeling that I came away with after "cramming" this morning on the subject of pharmacological history for the purpose of writing this essay. In reading a variety of accounts of the history of pharmacology, including one by medical historian John Parascandola, I got the sense of a field that had never had any self-doubts, that had never stopped to think that drugs like opium and coca were in any way different from quinine and streptomycin. "So, some of them involve human expectations and desires," the field's boosters seemed to say, "how hard can THAT BE?!" There was never a hint that the scientific method might be an inappropriate algorithm, let alone a biasing factor, in determining the efficacy of mind- and mood-affecting drugs.

There is something of the "bull in the China shop" in the story of American progress. We stride right in with our utilitarian purposes in mind, acting like we own the place, sure that we are dealing with all the relevant facts and that those who live in other cultures with other mindsets are just superstitious simpletons. Native-American activist Albert Hensley suggested the indigenous response to this viewpoint when he said the following about modern science's study of the peyote cactus:

It is utter folly for scientists to attempt to analyze this medicine. Can science analyze God's body? 4

The answer was clearly "yes" for Rudolf Buchheim, the "Father of Pharmacology," who wrote:

The pharmacologist has as little interest in the appearance of senna leaves as he has in the appearance of the case holding the scalpel which he uses for his operations. 5

But these are all arguments that I have made before, and in a wide variety of ways. I would like to end this essay by expressing a new concern that I have never yet broached when criticizing scientists for their bias on the subject of drugs, one that involves a philosophical bias that can arise even in cases where a drug is not, politically speaking, considered to be a "drug" at all in the negative acceptation of that word.

I am thinking of the heart medicine and beta-blocker known as Propranolol, which was first marketed in 1964. I myself was placed "on" this substance decades ago for the "off-label" use of fighting anxiety and I am still on that drug to this very day. I have since learned that there are concerns about the long-term use of such a drug for such a purpose, but that is not my concern today; that is another topic for which I invite other more qualified pundits to worry on my behalf. What strikes me today is that scientists never seem to have considered the philosophical questions raised by the use of this drug to fight anxiety. They never asked themselves, "Could the daily use of the drug decrease inspiration and motivation in the long run by numbing perceptions which, had they been left undampened, might have spurred the user on to make important changes in their life, maybe even to create works of art?" This is just one sample question to indicate the kinds of concerns that I feel are never even imagined by pharmacists, or medical scientists, or biochemists, when they opine on the safety of using such substances for achieving a specific purpose. I am not saying that I did not need SOMETHING for my anxiety at some point in my life; my question is, why are the answers from pharmaceutical companies always something that I have to take for life and which therefore beg important questions about the kind of person that they are helping me to be -- or else preventing me from becoming?

You'll notice that two of them were from Wikipedia, and that's always a problem for me. I always get distracted by the poor writing skills of the contributors on that platform. I myself am not exactly Hieronymus P. Shakespeare III, but you'd think that a group-edited page on a subject like "Materia Medica" would at least avoid blatant errors. To see what I mean, I invite you to see the changes below that I recommended to Wikipedia via their "talk" page for the article in question. I fear that these gaffes are just the tip of the problematic iceberg, since my goal in reading the article was simply to understand it, not to "catch the writer's out." But the errors in question just screamed out for my attention, as who should say, "Help! Flag us! We're obviously wrong, or silly, or BOTH!"

We should not be talking about the potential harm of drugs -- we should be talking about the well-established harm of drug PROHIBITION.

71% of the depressed have relapses after getting off their meds. Doctors blame this on depression, but increasing evidence suggests that these people are having withdrawal problems.

I might as well say that no one can ever be taught to ride a horse safely. I would argue as follows: "Look at Christopher Reeves. He was a responsible and knowledgeable equestrian. But he couldn't handle horses. The fact is, NO ONE can handle horses!"

In a sane world, we would learn to strategically fight drugs with drugs.

What is the end game of the drug warrior? A world in which no one wants drugs? That's not science. It's the drug-hating religion of Christian Science. You know, the American religion that outsources its Inquisition to drug-testing labs.

If there were no other problem with antidepressants, they would be wrong for the simple reason that they make a user dependent for life -- not as a bug (as in drugs like opium) but rather as a feature: that's how they "work," by being administered daily for a lifetime.

Drug Warriors never take responsibility for incentivizing poor kids throughout the west to sell drugs. It's not just in NYC and LA, it's in modest-sized towns in France. Find public housing, you find drug dealing. It's the prohibition, damn it!

Freud's real discovery was that drugs like cocaine could make psychiatry UNNECESSARY for the vast majority of people. The medical establishment hated the idea -- so they judged the drug based on its worst possible use!

Here are some political terms that are extremely problematic in the age of the drug war: "clean," "junk," "dope," "recreational"... and most of all the word "drugs" itself, which is as biased and loaded as the word "scab."

If Fentanyl kills, then alcohol slaughters. Drug prohibition is the real killer.
